Who we are
Staithes Surgery is part of the Whitby, Coast and Moors Primary Care Network (PCN). PCNs have been created to help integrate primary care with secondary and community services and provide care at greater scale. In doing this we can develop the services we offer to meet the needs of our local population.
Our new website has links that take you to our main PCN website to avoid duplication of information across each of our local GP practices. The ‘your health’ menu tab will take you to information about all the services in our local area, information about managing conditions and looking after you and your family.
You can find out more about our network by clicking on the ‘our network’ tab in the main menu. To get back to our individual practice site, simply select our practice name from the ‘practice websites’ menu tab.

How can I register for online services
You can now use the NHS app to book appointments with a GP, request repeat prescriptions for any medications you take regularly and to look at allergies and adverse reactions within your medical record. To register or download the NHS app please use the following link NHS APP
How do I complete a travel risk assessment
For travel immunisations and advice please complete the travel vaccination questionnaire and make an appointment to see the Practice Nurse, 6 to 8 weeks prior to travel. We recommend that you see us early in case you require a course of immunisations.
How do I register as a new patient?
We welcome new patients who wish to register with us.
To register with our surgery you will need to complete a registration form and a health questionnaire. The forms are available to collect from our surgery during our opening hours, or alternatively you may wish to download, print and complete a copy from our website. The appropriate form/s can be downloaded below or from our documents page:
- Patient registration form – GMS1
- Registration form for patients over five years of age
- Welcome pack – over five years of age
- Registration form for patients under five years of age
New patient check
Once you have provided us with your completed registration form, health questionnaire and identification please make an appointment with our Nurse or Health Care Assistant for a ‘new patient check’. As part of this check up, we will take basic measurements such as blood pressure, height and weight.
